I registered at CONCOR but have not received any
further information. |
| Reply: |
After registering, the relevant data are extracted from
your medical record and entered into the data bank. You are
given no further notification at this stage. |

| Question: |
I have registered but have still not been contacted
for blood withdrawal. |
| Reply: |
It may take a long time before you are contacted to make
an appointment for withdrawing blood for the DNA bank. In
some cases it may even take years. This is due to
restrictions regarding where and by whom blood may be
withdrawn. The number of hospitals for which CONCOR has
applied for approval by the local Medical Ethical Committee
for blood withdrawal is limited. Furthermore, for logistic
reasons it is not always possible for the research nurses to
personally withdraw blood from patients in all regions of
the country. |

When will I receive the results of my blood test? |
| Reply: |
In fact, you won’t receive any results. The DNA in your
blood is only analyzed for research purposes. It may occur,
however, that the researchers wish to extend their
investigations and need your further cooperation. You are
asked on the registration form to indicate whether or not
you are willing to participate in additional research. In
case test results lead to research findings of potential
interest for patients, your cardiologist will be informed
about these results on an aggregate (but not individual)
level. |

Are my data properly protected and completely safe? I
would not like, for example, insurance companies being able
to access my data. |
| Reply: |
Your data are protected appropriately and kept safe in
accordance with the Personal Data Protection Act. The only
person who has access to your data is the cardiologist who
is treating you. |

Why is it not possible to submit the registration
form on the CONCOR website directly via Internet? |
| Reply: |
The registration form is only valid with your signature
written on it. |
